Independent Living provides a broad scope of individualized services for adults with disabilities to create opportunities for a successful life in our community.
Independent Living Program Services
The Independent Living Program (ILP) of Broadscope Disability Services assists adults who have disabilities to live as independently as possible in the community. Services are provided to help the individual locate and utilize community resources and maintain and improve their health and living skills in order to achieve this goal.
Independent Living Components
Residential
Staff assist individuals with locating and maintaining affordable, accessible housing which meets their needs. Participants often utilize community resources such as subsidized housing. Program staff assist with application and annual review process.
Health
Staff assist individuals with locating medical providers and schedule and attend appointments to maintain continuity of care. Participants are also encouraged to engage in healthy lifestyle choices such as eating right and exercising. Medication management is also addressed by ILP staff.
Activities of Daily Living
Staff assist individuals with identifying needs in this area and provide skills training to improve independence as well as help establish services for meeting those needs with which participants require assistance. Services established may include Home Health, Supportive Home Care and Personal Care.
Communication
Staff assist individuals with communicating with family, friends, community contacts and also provide services in the areas of reviewing and responding to correspondence and advocacy.
Community Mobility
Staff assist individuals with developing a way to get to the places they want to go. This is done through establishing and utilizing Transit Plus eligibility as well as utilizing MCTS services or other resources specific to the individual. Also, ILP staff assist individuals with becoming familiar with places in their community they wish to visit.
Financial
Staff assist individuals to utilize any financial resources that may be available to them such as Social Security, Medicare, Medicaid and Long Term Care Funding-such as Family Care or IRIS. Broadscope can act as a participant’s Representative Payee if needed and also provides skills training in the areas of budgeting/money management.
Employment/Day Program/Education/Recreation
Staff assist individuals with locating and utilizing community resources to help them meet their goals. This may include assistance in finding work, going to school, or just spending their time in ways that are meaningful to them.
Support System/Safety
Staff assist individuals develop a support system which consists of natural and paid supports to ensure their needs are met and that they are as safe as possible. ILP staff address issues such as personal safety, emergency preparedness and self-advocacy. In the unfortunate event an program participant is the victim of a crime or other emergency situation, staff work with the individual in locating the proper assistance. Many participants utilize a Personal Emergency Response system and staff help set this system up and maintain current responder information.
